This patch removes the ARM architecture mach-pnx4008. No direct support or user
feedback since 2006. Acknowledgements from NXP/Philips and Linux arm-soc

The PNX4008 header file is using the generic gpio and gpiolib
namespace in <mach/gpio.h> yet the GPIO interface is not generic
at all so rename it to <mach/gpio-pnx4008.h>
This fixes a build failure in current -next: the includes were
changed from <mach/gpio.h> to <linux/gpio.h> but since this
platform isn't using generic gpio <linux/gpio.h> did not include
<mach/gpio.h> and things broke apart.

Convert arch/arm includes of mach/gpio.h and asm/gpio.h to linux/gpio.h
before we start consolidating the individual platform implementations
of the gpio header files.

This just leaves include/asm-arm/plat-* to deal with.

Remove includes of asm/hardware.h in addition to asm/arch/hardware.h.
Then, since asm/hardware.h only exists to include asm/arch/hardware.h,
update everything to directly include asm/arch/hardware.h and remove
asm/hardware.h.

Patch from Vitaly Wool
This patch moves GPIO-related defines and static inline funcs from include/asm-arm/arch-pnx4008/pm.h to include/asm-arm/arch-pnx4008/gpio.h.
Also, some more GPIO-related defines are added to include/asm-arm/arch-pnx4008/gpio.h as they are needed for the USB host driver (coming soon...)

It's basically the same as the previous one, but with the rmk's
comments taken into account.
