Never terribly functional or popular, plagued by hard to fix bugs the time
to say goodbye has more than arrived.

Existing code in there (get_tty(), etc.) is both severely
racy *and* pointless: ioctls in question have Linux equivalents
and there's no need to play silly buggers in irix_ioctl() -
just need to replace arguments and, in case of TIOCGSID,
deal with API differences - Linux one expects pid_t __user *
while Irix one does unsigned long __user *. BFD...

Remove includes of <linux/smp_lock.h> where it is not used/needed.
Suggested by Al Viro.
Builds cleanly on x86_64, i386, alpha, ia64, powerpc, sparc,
sparc64, and arm (all 59 defconfigs).

With the use of RCU in files structure, the look-up of files using fds can now
be lock-free. The lookup is protected by rcu_read_lock()/rcu_read_unlock().
This patch changes the readers to use lock-free lookup.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
