As announced in feature-remove-schedule.txt remove CONFIG_X86_OLD_MCE
This patch only removes code.
The ancient machine check code for very old systems that are not supported
by CONFIG_X86_NEW_MCE is still kept.

There are 2 headers:
arch/x86/include/asm/mce.h
arch/x86/kernel/cpu/mcheck/mce.h
and in the latter small header:
#include <asm/mce.h>
This patch move all contents in the latter header into the former,
and fix all files using the latter to include the former instead.

SMP, the machine check exception dispatches all logical processors within a
physical package to the machine-check exception handler, so the printk
within each handler outputs concurrently and makes the output unreadable.
