implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

We have almost the same code for mtrr cleanup and amd_bus checkup, and
this code will also be used in replacing bootmem with early_res,
so try to move them together and reuse it from different parts.
Also rename update_range to subtract_range as that is what the
function is actually doing.

This is part of the ops structure constification
effort started by Arjan van de Ven et al.
Benefits of this constification:
* prevents modification of data that is shared
(referenced) by many other structure instances
at runtime
* detects/prevents accidental (but not intentional)
modification attempts on archs that enforce
read-only kernel data at runtime
* potentially better optimized code as the compiler
can assume that the const data cannot be changed
* the compiler/linker move const data into .rodata
and therefore exclude them from false sharing

Makes use of skip_spaces() defined in lib/string.c for removing leading
spaces from strings all over the tree.
It decreases lib.a code size by 47 bytes and reuses the function tree-wide:

Also, while at it, if we see (*str && isspace(*str)), we can be sure to
remove the first condition (*str) as the second one (isspace(*str)) also
evaluates to 0 whenever *str == 0, making it redundant. In other words,
"a char equals zero is never a space".
Julia Lawall tried the semantic patch (http://coccinelle.lip6.fr) below,
and found occurrences of this pattern on 3 more files:

In some cases we can coalesce MTRR entries after cleanup; this may
allow us to have more entries. As such, introduce clean_sort_range to
to sort and coaelsce the MTRR entries.

The current MTRR code treats WP as a form of UC. This really isn't
desirable behaviour, except possibly in the case of severe MTRR
shortage. Disable this, to allow legitimate uses of WP to remain
unmolested.

The current bound checks for copy_from_user in the MTRR driver are
not as obvious as they could be, and gcc agrees with that.
This patch simplifies the boundary checks to the point that gcc can
now prove to itself that the copy_from_user() is never going past
its bounds.

mtr_aps_delayed_init was declared u32 and made global, but it only
ever takes boolean values and is only ever used in
arch/x86/kernel/cpu/mtrr/main.c. Declare it "static bool" and remove
external references.

SDM Vol 3a section titled "MTRR considerations in MP systems" specifies
the need for synchronizing the logical cpu's while initializing/updating
MTRR.
Currently Linux kernel does the synchronization of all cpu's only when
a single MTRR register is programmed/updated. During an AP online
(during boot/cpu-online/resume) where we initialize all the MTRR/PAT registers,
we don't follow this synchronization algorithm.
This can lead to scenarios where during a dynamic cpu online, that logical cpu
is initializing MTRR/PAT with cache disabled (cr0.cd=1) etc while other logical
HT sibling continue to run (also with cache disabled because of cr0.cd=1
on its sibling).
Starting from Westmere, VMX transitions with cr0.cd=1 don't work properly
(because of some VMX performance optimizations) and the above scenario
(with one logical cpu doing VMX activity and another logical cpu coming online)
can result in system crash.
Fix the MTRR initialization by doing rendezvous of all the cpus. During
boot and resume, we delay the MTRR/PAT init for APs till all the
logical cpu's come online and the rendezvous process at the end of AP's bringup,
will initialize the MTRR/PAT for all AP's.
For dynamic single cpu online, we synchronize all the logical cpus and
do the MTRR/PAT init on the AP that is coming online.

Yinghai noticed that i defined BIOS_BUG_MSG but added no
usage for it. The usage is to clean up this turd in generic.c:

Breaking printk lines in the middle looks ugly, is hard to read

found one system where cpu address line is 44bits, mtrr printout
is not right:

Li Zefan and Frederic pointed out the high_width could be -4 some how.
It turns out when phys_addr is 44bit, size_or_mask will be

Try to check low 32 bit, to get correct high_width.

Setting ->owner as done currently (pde->owner = THIS_MODULE) is racy
as correctly noted at bug #12454. Someone can lookup entry with NULL
->owner, thus not pinning enything, and release it later resulting
in module refcount underflow.
We can keep ->owner and supply it at registration time like ->proc_fops
and ->data.
But this leaves ->owner as easy-manipulative field (just one C assignment)
and somebody will forget to unpin previous/pin current module when
switching ->owner. ->proc_fops is declared as "const" which should give
some thoughts.
->read_proc/->write_proc were just fixed to not require ->owner for
protection.
rmmod'ed directories will be empty and return "." and ".." -- no harm.
And directories with tricky enough readdir and lookup shouldn't be modular.
We definitely don't want such modular code.
Removing ->owner will also make PDE smaller.
So, let's nuke it.
Kudos to Jeff Layton for reminding about this, let's say, oversight.

Ignore those strange ranges, and don't trim e820,

Impact: bug fix + BIOS workaround
BIOS is expected to clear the SYSCFG[MtrrFixDramModEn] on AMD CPUs
after fixed MTRRs are configured.
Some BIOSes do not clear SYSCFG[MtrrFixDramModEn] on BP (and on APs).
This can lead to obfuscation in Linux when this bit is not cleared on
BP but cleared on APs. A consequence of this is that the saved
fixed-MTRR state (from BP) differs from the fixed-MTRRs of APs --
because RdDram/WrDram bits are read as zero when
SYSCFG[MtrrFixDramModEn] is cleared -- and Linux tries to sync
fixed-MTRR state from BP to AP. This implies that Linux sets
SYSCFG[MtrrFixDramEn] and activates those bits.
More important is that (some) systems change these bits in SMM when
ACPI is enabled. Hence it is racy if Linux modifies RdMem/WrMem bits,
too.
(1) The patch modifies an old fix from Bernhard Kaindl to get
suspend/resume working on some Acer Laptops. Bernhard's patch
tried to sync RdMem/WrMem bits of fixed MTRR registers and that
helped on those old Laptops. (Don't ask me why -- can't test it
myself). But this old problem was not the motivation for the

(2) The more important effect is to fix issues on some more current systems.

The affected systems boot only using acpi=ht, acpi=off or
when the kernel is built with CONFIG_MTRR=n.
The acpi options prevent full enablement of ACPI. Obviously when
ACPI is enabled the BIOS/SMM modfies RdMem/WrMem bits. When
CONFIG_MTRR=y Linux also accesses and modifies those bits when it
needs to sync fixed-MTRRs across cores (Bernhard's fix, see (1)).
How do you synchronize that? You can't. As a consequence Linux
shouldn't touch those bits at all (Rationale are AMD's BKDGs which
recommend to clear the bit that makes RdMem/WrMem accessible).
This is the purpose of this patch. And (so far) this suffices to
fix (1) and (2).
I suggest not to touch RdDram/WrDram bits of fixed-MTRRs and
SYSCFG[MtrrFixDramEn] and to clear SYSCFG[MtrrFixDramModEn] as
suggested by AMD K8, and AMD family 10h/11h BKDGs.
BIOS is expected to do this anyway. This should avoid that
Linux and SMM tread on each other's toes ...

Impact: improve MTRR debugging messages
There's still inefficiencies suspected with the MTRR sanitizing
code, so make sure we get all the info we need from a dmesg.
- Remove unneeded mtrr_show
(It will only printout one time by first cpu, so it is no big deal.)
- Also print out directly from get_mtrr, because it doesn't update mtrr_state.

vmware not setting up any MTRRs for emulated CPUs:

I realized that the mtrr.show param cannot work, because
the code is processed much too early.
This patch:
- Declares mtrr.show as early_param
- Stays consistent with the previous param (which I doubt
that it ever worked), so mtrr.show=1 would still work
- Declares mtrr_show as initdata
