CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, and __devexit
from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

When 2R memories are found, the memory size should be multiplied
by two, otherwise, it will report half of the memory size:

"pvt->ambase" is a u64 datatype. The intent here is to fill the first
half in the first call to pci_read_config_dword() and the other half in
the second. Unfortunately the pointer math is wrong so we set the wrong
data.

In order to avoid loosing error events, it is desirable to group
error events together and generate a single trace for several identical
errors.
The trace API already allows reporting multiple errors. Change the
handle_error function to also allow that.
The changes at the drivers were made by this small script:

Remove the arch-dependent parameter, as it were not used,
as the MCE tracepoint weren't implemented. It probably doesn't
make sense to have an MCE-specific tracepoint, as this will
cost more bytes at the tracepoint, and tracepoint is not free.

Convert macros to a simpler style and enforce appropriate
format checking when not CONFIG_EDAC_DEBUG.
Use fmt and __VA_ARGS__, neaten macros.
Move some string arrays to the debugfx uses and remove the
now unnecessary CONFIG_EDAC_DEBUG variable block definitions.

As EDAC doesn't use struct device itself, it created a parent dev
pointer called as "pdev". Now that we'll be converting it to use
struct device, instead of struct devsys, this needs to be fixed.
No functional changes.

The fatal error channel bits point to a single channel, and not
to a range of channels. Fix the code to properly report it,
instead of printing messages like:

The logic there is broken: it basically creates two csrows for
each DIMM and assumes that all DIMM's are dual rank. Only one of
the csrows will contain the entire DIMM size. If single rank
memories are found, they'll be marked with 0 bytes.
The check if the AMB is present were also wrong.
Yet, as the error reports don't use the memory size in order to
credit an error to the right DIMM, that part of the driver seems
to work. That's why probably nobody detected the issue yet.
After this patch, the memory layout is now properly reported,
when debug mode is enabled, and the number of ranks per dimm is
now shown:

The number of pages is a dimm property. Move it to the dimm struct.
After this change, it is possible to add sysfs nodes for the DIMM's that
will properly represent the DIMM stick properties, including its size.
A TODO fix here is to properly represent dual-rank/quad-rank DIMMs when
the memory controller represents the memory via chip select rows.

Almost all edac drivers initialize csrow_info->first_page,
csrow_info->last_page and csrow_info->page_mask. Those vars are
used inside the EDAC core, in order to calculate the csrow affected
by an error, by using the routine edac_mc_find_csrow_by_page().
However, very few drivers actually use it:

There also a few other drivers that have their own calculus
formula internally using those vars.
All the others are just wasting time by initializing those
data.
While initializing data without using them won't cause any troubles, as
those information is stored at the wrong place (at csrows structure), it
is better to remove what is unused, in order to simplify the next patch.

On systems based on chip select rows, all channels need to use memories
with the same properties, otherwise the memories on channels A and B
won't be recognized.
However, such assumption is not true for all types of memory
controllers.
Controllers for FB-DIMM's don't have such requirements.
Also, modern Intel controllers seem to be capable of handling such
differences.
So, we need to get rid of storing the DIMM information into a per-csrow
data, storing it, instead at the right place.
The first step is to move grain, mtype, dtype and edac_mode to the
per-dimm struct.

These const tables are currently marked __devinitdata, but
Documentation/PCI/pci.txt says:
"o The ID table array should be marked __devinitconst; this is done
automatically if the table is declared with DEFINE_PCI_DEVICE_TABLE()."
So use DEFINE_PCI_DEVICE_TABLE(x).

The kernel already prints its build timestamp during boot, no need to
repeat it in random drivers and produce different object files each
time.

-EIO is not the only error code that pci_enable_device() may return, also
the set of errors can be enhanced in future. We should compare return
code with zero, not with concrete error value.

EDAC MC0: INTERNAL ERROR: channel-b out of range (4 >= 4)
Kernel panic - not syncing: EDAC MC0: Uncorrected Error (XEN) Domain 0 crashed: 'noreboot' set - not rebooting.
This happens because FERR_NF_FBD bit 28 is not updated on i5000. Due to
that, both bits 28 and 29 may be equal to one, returning channel = 3. As
this value is invalid, EDAC core generates the panic.

Allow csrows to properly initialize when the topology only has active
channels on 2 and 3. This new check allows proper detection and
initialization in this topology. Only checking the first mrt that
represented channels 0 and 1 is not sufficient.
I also fixed up the related debug information path. I can submit as a 2nd
patch if needed.

It turns out that edac_mc_del_mc will kobject_put the last kref on the
mci object.
If the timing is just right, that means that the mci object is freed
before before i5000_remove_one has a chance to free the resources
associated with it, causing a null pointer exceptions when unloading the
driver. Insert a kobject_{get,put} pair so that this doesn't happen.

Make the Thermal messages (temperature got past Tmid) be displayed only
once because:
1) it's the BIOS job to configure and handle the memory throttling
2) if the BIOS is broken or is aware about the condition, flooding the
system logs won't help anything.
3) According to the specification update for Intel 5000 MCHs, all the
revisions of this MCH have problems on the thermal sensors, making
not automatic (a.k.a. intelligent thermal throttling) impossible.

Update the i5000_edac messages, making everything pass through the EDAC
(so the log controls will work) and being more specific about the errors.
Also, it makes the miscellaneous errors optional and disabled by default.
As I didn't found anywhere information about M23ERR-M26ERR
(FERR_NF_THERMAL) on FERR_NF_FBD, I'm removing them.

I implemented opstate_init() as a inline function in linux/edac.h.
added calling opstate_init() to:

i82875p_edac.c
i82975x_edac.c
I wrote a fixed patch of
edac-fix-module-initialization-on-several-modules.patch,
and tested building 2.6.25-rc7 with applying this. It was succeed.
I think the patch is now correct.

Refactoring of sysfs code necessitated the refactoring of the edac_mc_alloc()
and edac_mc_add_mc() apis, of moving the index value to the alloc() function.
This patch alters the in tree drivers to utilize this new api signature.
Having the index value performed later created a chicken-and-the-egg issue.
Moving it to the alloc() function allows for creating the necessary sysfs
entries with the proper index number

This patch fixes some remnant spaces inserted by the use of Lindent.
Seems Lindent adds some spaces when it shoulded. These have been fixed.
In addition, goto targets have issues, these have been fixed
in this patch.

Move dev_name() macro to a more generic interface since it's not possible
to determine whether a device is pci, platform, or of_device easily.
Now each low level driver sets the name into the control structure, and
the EDAC core references the control structure for the information.
Better abstraction.

In the refactoring of edac_mc.c into several subsystem files,
the header file edac_mc.h became meaningless. A new header file
edac_core.h was created. All the files that previously included
"edac_mc.h" are changed to include "edac_core.h".

Provides a way for NMI reported errors on x86 to notify the EDAC
subsystem pending ECC errors by writing to a software state variable.
Here's the reworked patch. I added an EDAC stub to the kernel so we can
have variables that are in the kernel even if EDAC is a module. I also
implemented the idea of using the chip driver to select error detection
mode via module parameter and eliminate the kernel compile option.
Please review/test. Thx!
Also, I only made changes to some of the chipset drivers since I am
unfamiliar with the other ones. We can add similar changes as we go.

Eric Wollesen ported the Bluesmoke Memory Controller driver (written by Doug
Thompson) for the Intel 5000X/V/P (Blackford/Greencreek) chipset to the in
kernel EDAC model.
This patch incorporates the module for the 5000X/V/P chipset family
