Conversion of lmedm04 to dvb-usb-v2
Functional changes m88rs2000 tuner now uses all callbacks.
TODO migrate other tuners to the callbacks.
This patch is applied on top of [BUG] Re: dvb_usb_lmedm04 crash Kernel (rs2000)

There is a missing error handling when no firmware file found.
It seems that this is more of a problem with udev-182+.
However, so far udev-182 is only a problem on first ever plug.

Support for m88rs2000 module.
The driver also attempts to overcome occasional lock problems.
Call backs are now used for read_status, signal level and SNR.

Remove delays added for STV0288 frontend.
This patch significantly speeds up channel change and scan.
It requires patch 10161-STV0288 increase delay between carrier search.

Turn the pid filter off by caps option only.
This is so the full stream is passed to demuxer and not limited
by pid count.

This converts the drivers in drivers/media/* to use the
module_usb_driver() macro which makes the code smaller and a bit
simpler.
Added bonus is that it removes some unneeded kernel log messages about
drivers loading and/or unloading.

Reduce buffer size of Interupt urb to 128 bytes and polling
interval to 8.
The devices buffer appears to only handle a maxium of 40 bytes.
If the buffer is full a slowing effect is noticed causing occasionnal
dropped streaming packets.

Improve PID filtering and program register 20 correctly.
Make sure stream_on message is sent if streaming is off, otherwise
PIDs are not registered.

There are a couple of places where mutex_unlock() is called even
if mutex_lock_interruptible() failed. The patch fixes the issue.
Found by Linux Driver Verification project (linuxtesting.org).

usb_control_msg initiates (and waits for completion of) a dma transfer using
the supplied buffer. That buffer thus has to be seperately allocated on
the heap.
In lib/dma_debug.c the function check_for_stack even warns about it:

both the LME2510 and LME2510C.
Firmware dvb-usb-lme2510-s0194.fw and/or dvb-usb-lme2510c-s0194.fw
files are required.

The Remote Controller subsystem is meant to be used not only by Infra Red
but also for similar types of Remote Controllers. The core is not specific
to Infra Red. As such, rename:
- ir-core.h to rc-core.h
- IR_CORE to RC_CORE
- namespace inside rc-core.c/rc-core.h
To be consistent with the other changes.
No functional change on this patch.

This patch merges the ir_input_dev and ir_dev_props structs into a single
struct called rc_dev. The drivers and various functions in rc-core used
by the drivers are also changed to use rc_dev as the primary interface
when dealing with rc-core.
This means that the input_dev is abstracted away from the drivers which
is necessary if we ever want to support multiple input devs per rc device.
The new API is similar to what the input subsystem uses, i.e:
rc_device_alloc()
rc_device_free()
rc_device_register()
rc_device_unregister()
