CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

A recent commit "[media] v4l2: make vidioc_s_crop const" introduced
warnings in sh_vou. Fix them by cleanly separating writable and
read-only variables in cropping operations.

Write-only ioctls should have a const argument in the ioctl op.
Do this conversion for vidioc_s_crop.
Adding const for write-only ioctls was decided during the 2012 Media Workshop.

The remaining drivers are mostly platform drivers. Name the
dir to reflect it.
It makes sense to latter break it into a few other dirs.
