The fcoe.tc field is no longer used so remove it. After
the field is removed there is no need to keep fcoe_setapp()
around so remove it as well. And finally we can get rid
of some DCB #ifdef's in the fcoe code.

ixgbe: DCB, further cleanups to app configuration
Removed the getapp() routines from ixgbe because they are no
longer needed. It also allowed the set hardware routines to
use both IEEE 802.1Qaz app types and CEE app types. This
added code to do bit shifting in the IEEE case.
This patch reverts the checks and handles the IEEE case
from the setapp entry point. I prefer this because it
keeps the two paths from having to be aware of the DCB
mode. This resolves a bug where I missed setting the
selector bit in the IEEE spec value and left it in the
CEE value. Now that they are separate routines these types
of errors should not occur.

I am removing the requirement that Ntuple filters have the same
number of queues and requirements as ATR. As a result this change will
make it so that all the Ntuple flag does is disable ATR for now.
This change fixes an issue in which we were incorrectly re-enabling ATR
when we exited perfect filter mode. This was due to the fact that the
logic assumed RSS and DCB were mutually exclusive which is no longer the
case.
To correct this we just need to add a check to guarantee DCB is disabled
before re-enabling ATR.

ixgbe devices support different numbers of packet buffers either
8 or 4. Here we only allocate the minimal number of packet
buffers required to implement the net_devices number of traffic
classes.
Fewer traffic classes allows for larger packet buffers in
hardware. Also more Tx/Rx queues can be given to each
traffic class.
This patch is mostly about propagating the number of traffic
classes through the init path. Specifically this adds the 4TC
cases to the MRQC and MTQC setup routines. Also ixgbe_setup_tc()
was sanitized to handle other traffic class value.
Finally changing the number of packet buffers in the hardware
requires the device to reinit. So this moves the reinit work
from DCB into the main ixgbe_setup_tc() routine to consolidate
the reset code. Now dcbnl_xxx ops call ixgbe_setup_tc() to
configure packet buffers if needed.

With the app data on the kernel dcb_app list we no longer
need to specifically handle them in ixgbe for the CEE case.
So now we can remove app handling logic and check when the
hw is configured if the app data matches the hardware
configuration in set_hw_all().
If it does not match then we can reconfigure.

This patch adds support to use the priority assignment
table in the ieee_ets structure to map priorities to
traffic classes. Previously ixgbe only supported a
1:1 mapping. Now we can enable and disable hardware
DCB support when multiple traffic classes are actually
being used. This allows the default case all priorities
mapped to traffic class 0 to work in normal hardware
mode and utilize the full packet buffer.
This patch does not address putting the hardware in
4TC mode so packet buffer space may be underutilized
in this case. A follow up patch can address this
optimization. But at least we have the hooks to do
this now.
Also CEE will behave as it always has and map priorities
1:1 with traffic classes.

RSS had previously been disabled when DCB was enabled because
DCB was single queued per traffic class. Now that DCB implements
multiple Tx/Rx rings per traffic class enable RSS.
Here RSS hashes across the queues in the traffic class.

This enables multiple {Tx|Rx} rings per traffic class while in DCB
mode. In order to get this working as expected the tc_to_tx net
device mapping is configured as well as the prio_tc_map.
skb priorities are mapped across a range of queue pairs to get
a distribution per traffic class. The maximum number of
queue pairs used while in DCB mode is capped at 64. The hardware
max is actually 128 queues but 64 is sufficient for now and
allocating more seemed a bit excessive. It is easy enough to
increase the cap later if need be.
To get the 802.1Q priority tags inserted correctly ixgbe was
previously using the skb queue_mapping field to directly set
the 802.1Q priority. This no longer works because we have removed
the 1:1 mapping between queues and traffic class. Each ring
is aligned with an 802.1Qaz traffic class so here we add an
extra field to the ring struct to identify the 802.1Q traffic
class. This uses an extra byte of the ixgbe_ring struct

Now we can set the VLAN priority directly and it will be
correct. User space can indicate the 802.1Qaz priority
using the SO_PRIORITY setsocket() option and QOS layer will
steer the skb to the correct rings. Additionally using
the multiq qdisc with a queue_mapping action works as
well.

Implement ieee_setapp dcbnl ops in ixgbe. This is required
to setup FCoE which requires dedicated resources. If the
app data is not for FCoE then no action is taken in ixgbe
except to add it to the dcb_app_list.

This implements dcbnl get and set capabilities ops. The
devices supported by ixgbe can be configured to run in
IEEE or CEE modes but not both.
With the DCBX set capabilities bit we add an explicit
signal that must be used to toggle between these modes.
This patch adds logic to fail the CEE command set_hw_all()
which programs the device with a CEE configuration if
the CEE caps bit is not set. Similarly, IEEE set
commands will fail if the IEEE caps bit is not set. We
allow most CEE config set commands to occur because they
do not touch the hardware until set_hw_all() is called.
The one exception to the above is the {set|get}app routines.
These must always be protected by caps bits to ensure
side effects do not corrupt the current configured mode.
By requiring the caps bit to be set correctly we can
maintain a consistent configuration in the hardware
for CEE or IEEE modes and prevent partial hardware
configurations that may occur if user space does
not send a complete IEEE or CEE configurations.
It is expected that user space will signal a DCBX mode
before programming device.

This removes the RESET bit previously used to force a device
reset when DCB bandwidth configurations were changed. This can
now be done dynamically without a reset so the bit is no longer
needed. The only remaining operations that force a device reset
are DCB enable/disable and FCoE application priority changes.
DCB enable/disable is a hardware requirement.

The 82599 and 82598 devices do not require hardware resets to
configure CEE pg settings. This patch changes DCB configuration
to set the CEE pg values directly from the dcbnl ops routine.
This reduces the number of resets seen on the wire and allows
LLDP to reach a steady state faster.

Currently the routines that configure the HW for DCB require a
ixgbe_dcb_config structure. This structure was designed to support
the CEE standard and does not match the IEEE standard well.
This patch changes the HW routines in ixgbe_dcb_8259x.{ch} to use
raw pfc and bandwidth values. This requires some parsing of the DCB
configuration but makes the HW routines independent of the data
structure that contains the DCB configuration.
The primary advantage to doing this is we can do HW setup directly
from the 802.1Qaz ops without having to arbitrarily encapsulate this
data into the CEE structure.

If the FCoE priority is not changing do not set the RESET and
APP_UPCHG bits. This causes unneeded HW resets and which can
cause unneeded LLDP frames and negotiations.
The current check is not sufficient because the FCoE priority
can change twice during a negotiation which results in the
bits being set. This occurs when the switch changes the
priority or when the link is reset with switches that do not
include the APP priority until after PFC has been negotiated.
This results in set_app being called with the local APP
priority. Then the negotiation completes and set_app
is called again with the peer APP priority. The check
fails so the device is reset and the above occurs again
resulting in an endless loop of resets.
By only resetting the device if the APP priority has really
changed we short circuit the loop.

This patch removes from drivers/net/ all the unnecessary
return; statements that precede the last closing brace of
void functions.
It does not remove the returns that are immediately
preceded by a label as gcc doesn't like that.
It also does not remove null void functions with return.
Done via:

Call ixgbe_copy_dcb_cfg() earlier in the ixgbe_dcbnl_set_all() so that
we can learn if this is going to fail as early as possible. Previously,
ixgbe_down or ixgbe_close were being called before this check and the
IXGBE_RESETTING bit was being set and cleared. Worse if this failed
the corresponding ixgbe_up/ndo_open would not called.

This patch adds support for multiple transmit queues to the Fiber Channel
over Ethernet (FCoE) feature found in 82599. Currently, FCoE has multiple
Rx queues available, along with a redirection table, that helps distribute
the I/O load across multiple CPUs based on the FC exchange ID. To make
this the most effective, we need to provide the same layout of transmit
queues to match receive.
Particularly, when Data Center Bridging (DCB) is enabled, the designated
traffic class for FCoE can have dedicated queues for just FCoE traffic,
while not affecting any other type of traffic flow.

This adds support to the net_device_ops.ndo_fcoe_enable/disable for 82599. This
consequently allows us to dynamically turn FCoE offload feature on or off
upon incoming calls to ndo_fcoe_enable/disable. When this happens, FCoE offload
features are enabled/disabled accordingly, and this is regardless of whether
DCB being turned on or not.

Currently, FCoE offload feature is turned on when the kernel config has
CONFIG_FCOE or CONFIG_FCOE_MODULE set. However, we really want to turn
FCoE offload on when there is FCoE traffic passing and turn it off when
it's just LAN traffic. Since FCoE depends on a lossless network provided
by DCB, this allows us to have FCoE turned on/off when user turns on DCB
using dcbtool.

Fix coexistence of Fiber Channel over Ethernet (FCoE) and Flow Director (FDIR)
in 82599 and remove the disabling of FDIR when FCoE is enabled.
Currently, FDIR is turned off when FCoE is enabled under the assumption that
FCoE is always enabled with DCB being turned on. However, FDIR does not have
to be turned off all the time when FCoE is enabled since FCoE can be enabled
without DCB being turned on, e.g., use link pause only. This patch makes sure
that when DCB is turned on or off, FDIR is turned on or off correspondingly;
and when FCoE is enabled, it does not disable FDIR, rather, it will have FDIR
set up properly so FCoE and FDIR can coexist regardless of DCB being on or off.

When running in DCB mode, switching between link flow control and priority
flow control shouldn't need to reset the hardware. This removes that
reset.
This also extends the set_all() dcbnl callback to return a value indicating
that the HW config changed, however a reset was not required.

82599 supports using either link flow control or priority flow control when
in DCB mode. The dcbnl interface already supports sending down
configurations through rtnetlink that can enable LFC when DCB is enabled,
so the driver should take advantage of this.
82598 does not support using LFC when DCB is enabled, so explicitly disable
it when we're in DCB mode. This means we always run in PFC mode when DCB
is enabled.

After acquiring the SAN MAC address from the EEPROM, we need to program it
into one of the RARs. Also, DCB will use this MAC address to run DCBX
commands, so it doesn't have to play musical MAC addresses when things like
bonding enter the picture. So we need to return the MAC address through
the netlink interface to userspace.
This also moves the init_rx_addrs() call out of start_hw() and into
reset_hw(). We shouldn't try to read any of the RAR information before
initializing our internal accounting of the RAR table, which was what
was happening.

Currently the q_vectors are being allocated statically inside of the
adapter struct. This increases the overall size of the adapter struct when
we can easily allocate the vectors dynamically. This patch changes that
behavior so that the q_vectors are allocated dynamically and the napi
structures are automatically allocated inside of the q_vectors as needed.

The priority flow control settings from the netlink layer aren't taking
effect in the base driver. The boolean pfc_mode_enable in the dcb_config
struct isn't being set, so the hardware configuration code is never
reached.

When changing DCB parameters, ixgbe needs to have the MAC reset. The way
the flow control code is setup today, PFC will be disabled on a reset.
This patch adds a new flow control type for PFC, and then has the netlink
layer take care of toggling which type of flow control to enable.

This patch adds the DCB (Data Center Bridging) support for 82599 hardware.
This is similar to how the 82598 DCB code works.
This patch also removes the BCN (Backwards Congestion Notification) netlink
configuration code from the driver. BCN was a pre-standard congestion
notification framework, and was not what the IEEE body decided upon for
standard congestion management. QCN (802.1Qau), Quantized Congestion
Notification is the accepted standard, which is not supported by 82599,
hence we remove the support altogether.
