When we are in a recover process from a chip fatal error,
driver should skip over execution of mailbox commands during
resetting chip.

Removing usage of rtnl_lock() to protect firmware interface registers.
These registers are accessed in some worker threads and can create a
deadlock if rtnl_lock is taken by upper layers while the worker is still
pending.
We remove rtnl_lock and use a driver mutex just while mailboxes are
accessed.

By default we add firmware information to ethtool get regs.
Optionally firmware info can instead be sent to log.

In some cases the firmware may be dead. Instead we dump the firmware
parameters and then restart it.

This replace the PCI DMA state API (include/linux/pci-dma.h) with the
DMA equivalents since the PCI DMA state API will be obsolete.
No functional change.

Use local copy of current mac address when initializing. In bonding
testing we saw cases where dev_addr was out of data causing failover
errors.

Add periodic heartbeat register read to trigger the eeh
recovery process.
We see cases where an eeh error was injected and the slot was
suspended. An asic access attempt is required to flush the recovery process,
but without interrupts the process can stall.
Adding this periodic register read causes the recovery process to begin.

Fix crash where resources are freed twice on an eeh recovery failure.
If eeh recovery fails we set a flag to indicate to close() that
resources have been freed.

Adding the infrstructure and basic data for the firmware
core dump.
The firmware coredump is turned OFF by default. There will be no
memory allocations for data dumps to the log.

Using 4-byte aligned headers is problematic for some architectures.
Since qlge uses 4-byte aligned rx buffers we split headers for these
architectures into a separate buffer and then recopy to align on 2-byte boundary.

The mailbox command process would only process a maximum of 5 unrelated
firmware events while waiting for it's command completion status.
It should process an unlimited number of events while waiting for a maximum of 5 seconds.

Change RX large buffer size based on MTU. If pages are larger
than the MTU the page is divided up into multiple chunks and passed to
the hardware. When pages are smaller than MTU each RX buffer can
contain be comprised of up to 2 pages.

Add wait for NIC fifo and MGMNT fifo to empty before applying reset.
Otherwise broken frames can be processed by management processor and
cause it to hang.

Get rid of spinlock and private mutex usage for exclusive access to the
HW semaphore register. rtnl_lock already creates exclusive access to
this register in all driver API.
Add rtnl to firmware worker threads that also use the HW semaphore register.

TX completions were running in a workqueue queued by the ISR. This
patch moves the processing of TX completions to an existing RSS NAPI
context.
Now each irq vector runs NAPI for one RSS ring and one or more TX
completion rings.

Currently we have three types of RX rings.
1) Default ring - services rx_ring for broadcast/multicast, handles
firmware events, and errors.
2) TX completion ring - handles only outbound completions.
3) RSS ring - handles only inbound completions.
This patch gets rid of the default ring type and moves it's functionality
into the first RSS ring. This makes better use of MSIX vectors since
they are a limited resource on some platforms.
