Don't use create_proc_read_entry() as that is deprecated, but rather use
proc_create_data() and seq_file instead.

create_proc_entry() shouldn't be used. Rather proc_create_data() should be
used. The proc_write() function is only used by #if'd out code, so delete it
for now.

Use strlcpy where possible to ensure the string is \0 terminated.
Use always sizeof(string) instead of 32, ETHTOOL_BUSINFO_LEN
and custom defines.
Use snprintf instead of sprint.
Remove unnecessary inits of ->fw_version
