Start using the control module driver for powering on the PHY and for
writing to the mailbox instead of writing to the control module
registers on their own.

In order to make single zImage work for ARM architecture,
we need to make sure we don't depend on private headers.
Move USB platform_data to <linux/platform_data/omap-usb.h>
and add a minimal drivers/mfd/usb-omap.h.

The glue layer should directly write to mailbox register (present in
control module) instead of calling phy layer to write to mailbox
register. Writing to mailbox register notifies the core of events like
device connect/disconnect.
Currently writing to control module register is taken care in this
driver which will be removed once the control module driver is in place.

Move the remaining headers under plat-omap/include/mach
to plat-omap/include/plat. Also search and replace the
files using these headers to include using the right path.
This was done with:
