Because gen6_gt_force_wake_{get,put} should already be responsible for
getting/putting runtime PM. If we keep these calls, debugfs will not
be testing the get/put calls of the forcewake functions.

If runtime PM is enabled and we unset all modes, we will runtime
suspend after __intel_set_mode() , then function
intel_modeset_check_state() will try to read the HW state while it is
suspended and trigger lots of WARNs because it shouldn't be reading
registers.
So on this patch we make intel_ddi_connector_get_hw_state() return
false in case the power domain is disabled, and we also make
intel_display_power_enabled() return false in case the device is
suspended. Notice that we can't just use
intel_display_power_enabled_sw() because while the driver is being
initialized the power domain refcounts are not reflecting the real
state of the hardware.
Just for reference, I have previously published an alternate patch for
this problem, called "drm/i915: get runtime PM at intel_set_mode".

At i915_display_info, don't call cursor_position() for a disabled
CRTC, since the CRTC may be on a powered down pipe, and this will
cause "Unclaimed register before interrupt" error messages.

panel VDD since the device is suspended.
We started getting WARNs about this problem since the patch that
started checking if we're suspended while reading registers.

This commit fixes a build error reported by Fengguang, that is
triggered when CONFIG_NETWORK_PHY_TIMESTAMPING is not set:
ERROR: "ptp_classify_raw" [drivers/net/ethernet/oki-semi/pch_gbe/pch_gbe.ko] undefined!
The fix is to introduce its own file for the PTP BPF classifier,
so that PTP_1588_CLOCK and/or NETWORK_PHY_TIMESTAMPING can select
it independently from each other. IXP4xx driver on ARM needs to
select it as well since it does not seem to select PTP_1588_CLOCK
or similar that would pull it in automatically.
This also allows for hiding all of the internals of the BPF PTP
program inside that file, and only exporting relevant API bits
to drivers.
This patch also adds a kdoc documentation of ptp_classify_raw()
API to make it clear that it can return PTP_CLASS_* defines. Also,
the BPF program has been translated into bpf_asm code, so that it
can be more easily read and altered (extensively documented in [1]).
In the kernel tree under tools/net/ we have bpf_asm and bpf_dbg
tools, so the commented program can simply be translated via
`./bpf_asm -c prog` where prog is a file that contains the
commented code. This makes it easily readable/verifiable and when
there's a need to change something, jump offsets etc do not need
to be replaced manually which can be very error prone. Instead,
a newly translated version via bpf_asm can simply replace the old
code. I have checked opcode diffs before/after and it's the very
same filter.

So far force_wake_timer was only used by gen6_gt_force_wake_put. Since
we always had balanced gen6_gt_force_wake_get/put calls, we could
guarantee balanced calls to intel_runtime_pm_get/put.
Commit 8232644ccf099548710843e97360a3fcd6d28e04, "drm/i915: Convert
the forcewake worker into a timer func" started scheduling the
force_wake_timer at gen6_read, which resulted in an unbalanced
runtime_pm refcount.
So this commit just reverts to the old behavior until we can find a
proper way to used delayed force_wake from the register read/write
macros without leaving the runtime_pm refcounts unbalanced and without
runtime suspending the driver while forcewake is active.

Atm we reserve/allocate and free the power context during GT power
enable/disable time. There is no need to do this, we can reserve/allocate
the buffer once during driver loading and free it during driver cleanup.
The re-reservation can also fail in case the driver previously manages to
allocate something on the given fixed address.
The buffer isn't exepected to move even if allocated by the BIOS, for
safety add an assert to check this assumption.
This also fixed a bug for Ville, where re-reserving the context failed
during a GPU reset (I assume because something else got allocated on its
fixed address).

"err" is always zero at this point so we always unregister and free the
mdio_bus before returning success. This seems like left over code and
I have deleted it.

When using the "separate_tx_channels=1" module parameter, the TX queues are
initially numbered starting from the first TX-only channel number (after all the
RX-only channels). efx_set_channels() renumbers the queues so that they are
indexed from zero.
On EF10, the TX queues need to be relabelled in this way before calling the
dimension_resources NIC type operation, otherwise the TX queue PIO buffers can be
linked to the wrong VIs when using "separate_tx_channels=1".

When netback discovers frontend is sending malformed packet it will
disables the interface which serves that frontend.
However disabling a network interface involving taking a mutex which
cannot be done in softirq context, so we need to defer this process to
kthread context.
This patch does the following:
1. introduce a flag to indicate the interface is disabled.
2. check that flag in TX path, don't do any work if it's true.
3. check that flag in RX path, turn off that interface if it's true.
The reason to disable it in RX path is because RX uses kthread. After
this change the behavior of netback is still consistent -- it won't do
any TX work for a rogue frontend, and the interface will be eventually
turned off.
Also change a "continue" to "break" after xenvif_fatal_tx_err, as it
doesn't make sense to continue processing packets if frontend is rogue.
This is a fix for XSA-90.

Make sure that vxlan_get_rx_port() is present in the kernel build in a manner
consistent with mlx4, else mlx4 can be made built-in where vxlan a module and
the phase of the build linking fails. Add CONFIG_MLX4_EN_VXLAN for that.
Also, #ifdef the advertizement and implementation of the mlx4 vxlan ndo
calls and related code under this config directive.

Introduce a CONFIG_BE2NET_VXLAN define to control be2net's build
dependency on the VXLAN driver.
Without this fix, the kernel build fails when VxLAN driver is
selected to be built as a module while be2net is built-in.

Pull ACPI and power management updates from Rafael Wysocki:
"The majority of this material spent some time in linux-next, some of
it even several weeks. There are a few relatively fresh commits in
it, but they are mostly fixes and simple cleanups.
ACPI took the lead this time, both in terms of the number of commits
and the number of modified lines of code, cpufreq follows and there
are a few changes in the PM core and in cpuidle too.
A new feature that already got some LWN.net's attention is the device
PM QoS extension allowing latency tolerance requirements to be
propagated from leaf devices to their ancestors with hardware
interfaces for specifying latency tolerance. That should help systems
with hardware-driven power management to avoid going too far with it
in cases when there are latency tolerance constraints.
There also are some significant changes in the ACPI core related to
the way in which hotplug notifications are handled. They affect PCI
hotplug (ACPIPHP) and the ACPI dock station code too. The bottom line
is that all those notification now go through the root notify handler
and are propagated to the interested subsystems by means of callbacks
instead of having to install a notify handler for each device object
that we can potentially get hotplug notifications for.
In addition to that ACPICA will now advertise "Windows 2013"
compatibility for _OSI, because some systems out there don't work
correctly if that is not done (some of them don't even boot).
On the system suspend side of things, all of the device suspend and
resume callbacks, except for ->prepare() and ->complete(), are now
going to be executed asynchronously as that turns out to speed up
system suspend and resume on some platforms quite significantly and we
have a few more optimizations in that area.
Apart from that, there are some new device IDs and fixes and cleanups
all over. In particular, the system suspend and resume handling by
cpufreq should be improved and the cpuidle menu governor should be a
bit more robust now.
Specifics:
- Device PM QoS support for latency tolerance constraints on systems
with hardware interfaces allowing such constraints to be specified.
That is necessary to prevent hardware-driven power management from
becoming overly aggressive on some systems and to prevent power
management features leading to excessive latencies from being used
in some cases.
- Consolidation of the handling of ACPI hotplug notifications for
device objects. This causes all device hotplug notifications to go
through the root notify handler (that was executed for all of them
anyway before) that propagates them to individual subsystems, if
necessary, by executing callbacks provided by those subsystems
(those callbacks are associated with struct acpi_device objects
during device enumeration). As a result, the code in question
becomes both smaller in size and more straightforward and all of
those changes should not affect users.
- ACPICA update, including fixes related to the handling of _PRT in
cases when it is broken and the addition of "Windows 2013" to the
list of supported "features" for _OSI (which is necessary to
support systems that work incorrectly or don't even boot without
it). Changes from Bob Moore and Lv Zheng.
- Consolidation of ACPI _OST handling from Jiang Liu.
- ACPI battery and AC fixes allowing unusual system configurations to
be handled by that code from Alexander Mezin.
- New device IDs for the ACPI LPSS driver from Chiau Ee Chew.
- ACPI fan and thermal optimizations related to system suspend and
resume from Aaron Lu.
- Cleanups related to ACPI video from Jean Delvare.
- Assorted ACPI fixes and cleanups from Al Stone, Hanjun Guo, Lan
Tianyu, Paul Bolle, Tomasz Nowicki.
- Intel RAPL (Running Average Power Limits) driver cleanups from
Jacob Pan.
- intel_pstate fixes and cleanups from Dirk Brandewie.
- cpufreq fixes related to system suspend/resume handling from Viresh

The removal of these methods is compensated for by code changes to
.map_sg to insure that the vanilla sg_dma_address() and sg_dma_len()
will do the same thing as the equivalent former ib_sg_dma_address()
and ib_sg_dma_len() calls into the drivers.
The introduction of this patch required that the struct
ipath_dma_mapping_ops be converted to a C99 initializer.

Add an iser info print with the local/remote QP information carried
out when the connection is established. While here, fix a little
leftover from the T10 work and set a debug print to be carried in
debug and not info level.

The iscsi stack has existing mechanisms to link back and forth between
the iscsi connection and the iscsi transport (e.g iser/tcp) connection.
This is done through a dd_data pointer field in struct iscsi_conn
which can be set to point to the transport connection, etc.
The iscsi_iser_conn structure was used to get this linking done in
another way, which is uneeded and adds extra complication to the iser
code, so we just remove it.

The iser disconnection flow isn't done before all the inflight
recv/send buffers posted to the QP are either flushed or normally
completed to the CQ that serves this connection. The condition check
is done in iser_handle_comp_error().
Currently, it's possible for the send buffer completion that makes the
posted send buffers counter reach zero to be polled in the drain tx
call, which is after the rx cq is fully drained. Since this
completion might be not an error one (for example, it might be a
completion of the logout request iSCSI PDU) we will skip
iser_handle_comp_error(). So the connection will never terminate from
the iscsi stack point of view, and we hang.
To resolve this race, do the draining of the tx cq before the loop on
the rx cq.

My static checker complains that the sprintf() here can overflow.

error: format string overflow. buf_size: 32 length: 69
This seems like a valid complaint. The "dev->pdev->bus->name" string
can be 48 characters long. I just made the buffer 80 characters instead
of 69 and I changed the sprintf() to snprintf().

Running with DMA_API_DEBUG enabled and not checking for DMA mapping
errors triggers a kernel stack trace with "DMA-API: device driver
failed to check map error" message. Add these checks to the MAD
module, both to be be more robust and also eliminate these
false-positive stack traces.

In case of error when writing to userspace, function ehca_create_cq()
does not set an error code before following its error path.
This patch sets the error code to -EFAULT when ib_copy_to_udata()
fails.
This was caught when using spatch (aka. coccinelle)
to rewrite call to ib_copy_{from,to}_udata().
