After moving the the include files there were a few clean-ups:
1) Some files used #include <asm-ia64/xyz.h>, changed to <asm/xyz.h>
2) Some comments alerted maintainers to look at various header files to
make matching updates if certain code were to be changed. Updated these
comments to use the new include paths.
3) Some header files mentioned their own names in initial comments. Just
deleted these self references.

introduce pv_cpu_ops to paravirtualize privleged instructions
which are defined by ia64 intrinsics.
make them indirect C function calls by introducing function
tables, pv_cpu_ops.

Fixes userspace build errors when linux/ipv6.h is included such as in the
dhcpv6 package under fedora. Likely causes other userspace build errors as
well. I found this in akpm's tree from 2.6.18 but could not find any case
of anyone proposing it for the main tree.

__clear_bit_unlock does not need to perform atomic operations on the
variable. Avoid a cmpxchg and simply do a store with release semantics.
Add a barrier to be safe that the compiler does not do funky things.

The ia64-version of fls() never worked as intended (the bitnumbering
was off by 1 and fls(0) was undefined). This patch fixes the problem
by using a popcnt-based fls(), which on McKinley-derived cores is
slightly faster than both ia64_fls() and generic_fls(). The resulting
code, however, is bigger (7-8 bundles instead of about 3 bundles).
Also switch ia64_popcnt() to __builtin_popcountl() for GCC v3.4 or
newer since the compiler can predicate that and schedule it better.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
