virt_addr_valid() calls __pa(), which calls __phys_addr(). With
CONFIG_DEBUG_VIRTUAL=y, __phys_addr() will kill the kernel if the
address *isn't* valid. That's clearly wrong for virt_addr_valid().
We also incorporate the debugging checks into virt_addr_valid().

Track the memtype for RAM pages in page struct instead of using the
memtype list. This avoids the explosion in the number of entries in
memtype list (of the order of 20,000 with AGP) and makes the PAT
tracking simpler.
We are using PG_arch_1 bit in page->flags.
We still use the memtype list for non RAM pages.

On 32-bit architectures PAGE_ALIGN() truncates 64-bit values to the 32-bit
boundary. For example:
u64 val = PAGE_ALIGN(size);
always returns a value < 4GB even if size is greater than 4GB.
The problem resides in PAGE_MASK definition (from include/asm-x86/page.h for
example):
#define PAGE_SHIFT 12
#define PAGE_SIZE (_AC(1,UL) << PAGE_SHIFT)
#define PAGE_MASK (~(PAGE_SIZE-1))
...
#define PAGE_ALIGN(addr) (((addr)+PAGE_SIZE-1)&PAGE_MASK)
The "~" is performed on a 32-bit value, so everything in "and" with
PAGE_MASK greater than 4GB will be truncated to the 32-bit boundary.
Using the ALIGN() macro seems to be the right way, because it uses
typeof(addr) for the mask.
Also move the PAGE_ALIGN() definitions out of include/asm-*/page.h in
include/linux/mm.h.

Add an hugepagesz=... option similar to IA64, PPC etc. to x86-64.
This finally allows to select GB pages for hugetlbfs in x86 now that all
the infrastructure is in place.

This patch is the result of an automatic script that consolidates the
format of all the headers in include/asm-x86/.
The format:
1. No leading underscore. Names with leading underscores are reserved.
2. Pathname components are separated by two underscores. So we can
distinguish between mm_types.h and mm/types.h.
3. Everything except letters and numbers are turned into single
underscores.

Rusty, in his peevish way, complained that macros defining constants
should have a name which somewhat accurately reflects the actual
purpose of the constant.
Aside from the fact that PTE_MASK gives no clue as to what's actually
being masked, and is misleadingly similar to the functionally entirely
different PMD_MASK, PUD_MASK and PGD_MASK, I don't really see what the
problem is.
But if this patch silences the incessent noise, then it will have
achieved its goal (TODO: write test-case).

Change a15af1c9ea2750a9ff01e51615c45950bad8221b 'x86/paravirt: add
pte_flags to just get pte flags' removed lguest's private pte_flags()
in favor of a generic one.
Unfortunately, the generic one doesn't filter out the non-flags bits:
this results in lguest creating corrupt shadow page tables and blowing
up host memory.
Since noone is supposed to use the pfn part of pte_flags(), it seems
safest to always do the filtering.

Add pte_flags() to extract the flags from a pte. This is a special
case of pte_val() which is only guaranteed to return the pte's flags
correctly; the page number may be corrupted or missing.
The intent is to allow paravirt implementations to return pte flags
without having to do any translation of the page number (most notably,
Xen).

Fix the warning:
include2/asm/pgtable.h: In function `pte_modify':
include2/asm/pgtable.h:290: warning: left shift count >= width of type
On 32-bit PAE the virtual and physical addresses are both 32-bits,
so it ends up evaluating 1<<32. Do the shift as a 64-bit shift then
cast to the appropriate size. This should all be done at compile time,
and so have no effect on generated code.

Define PTE_MASK so that it contains a meaningful value for all x86
pagetable configurations. Previously it was defined as a "long" which
means that it was too short to cover a 32-bit PAE pte entry.
It is now defined as a pteval_t, which is an integer type long enough
to contain a full pte (or pmd, pud, pgd).
This fixes an Xorg crash on 32-bit x86 with PAE due to corruption of the
NX bit in mprotect due to the incorrect type/value of PTE_MASK reported
by Hugh Dickins:
"Yes, thanks Jeremy: I've checked that each stage builds and runs X on
my boxes here, x86_32 and x86_32+PAE and x86_64. (So even 1/8 is
enough to fix the PAT pte_modify issue, though 2/8 then fixes
compiler warnings.)"

This patch introduces a restriction on /dev/mem: Only non-memory can be
read or written unless the newly introduced config option is set.
The X server needs access to /dev/mem for the PCI space, but it doesn't need
access to memory; both the file permissions and SELinux permissions of /dev/mem
just make X effectively super-super powerful. With the exception of the
BIOS area, there's just no valid app that uses /dev/mem on actual memory.
Other popular users of /dev/mem are rootkits and the like.
(note: mmap access of memory via /dev/mem was already not allowed since
a really long time)
People who want to use /dev/mem for kernel debugging can enable the config
option.
The restrictions of this patch have been in the Fedora and RHEL kernels for
at least 4 years without any problems.

Even on 32bit 2MB pages can map more memory than is in the true
max_low_pfn if end_pfn is not highmem and not aligned to 2MB.
Add a end_pfn_map similar to x86-64 that accounts for this
fact. This is important for code that really needs to know about
all mapping aliases.

64bit uses end_pfn_map and 32bit uses max_low_pfn. There are several
files which have #ifdef'ed defines which map either to end_pfn_map or
max_low_pfn. Replace this by a universal define and clean up all the
other instances.

Make sure pte_t, whatever its definition, has a pte element with type
pteval_t. This allows common code to access it without needing to be
specifically parameterised on what pagetable mode we're compiling for.
For 32-bit, this means that pte_t becomes a union with "pte" and "{
pte_low, pte_high }" (PAE) or just "pte_low" (non-PAE).

This patch:
1. Defines arch-specific types for the contents of a pagetable entry.
That is, 32-bit entries for 32-bit non-PAE, and 64-bit entries for
32-bit PAE and 64-bit. However, even though the latter two are the
same size, they're defined with different types in order to retain
compatibility with printk format strings, etc.
2. Defines arch-specific pte_t. This is different because 32-bit PAE
defines it in two halves, whereas 32-bit PAE and 64-bit define it as a
single entry. All the other pagetable levels can be defined in a
common way. This also defines arch-specific pte_val/make_pte functions.
3. Define PAGETABLE_LEVELS for each architecture variation, for later use.
4. Define common pagetable entry accessors in a paravirt-compatible
way. (64-bit does not yet use paravirt-ops in any way).
5. Convert a few instances of using a *_val() as an lvalue where it is
no longer a macro. There are still places in the 64-bit code which
use pte_val() as an lvalue.
